When A Maiar is killed its only the body that is destroyed after a very long period they will be able to take form again at the cost of a loss of power. The exceptions to the power loss here are Sauron because he placed most of his power in the one ring and Gandalf because of a bit of devine intervention. Eru (god) had been watching the five wizards and like the plan the Valar had but when Gandalf died it hit a big snag. Since Gandalf was the only one that had remained true to the cause. Saruman the white had defected to the enemy; Randgast the Brown was having fun with nature and its creatures and had lost interest in the mission and the two blue wizards, no one had heard anything from and it is not known what happened to them after they went into the East. So Eru decided the best course of action was to send Gandalf back but expand on the plan of the Valar by promoting him to the head of the order and upgrading him slightly but this would have a price, since it went against the natural order of things and that price was short term memory loss of some things. He was picked up by an eagle and arrived in Lorien one day after the fellowship left by boat and by the time he spoke to the gang he would have remembered most stuff. Gandalf had so many names in middle-earth I guess that was at the back of his mind when remembering stuff. The name Gandalf was also mostly used in the north. The elves and men of the south tend to call him Mithrandir, so if anyone was to have talked to him they probably would have called him that.
