Yeah, FSOD and FSOD-X are real, it's happened to alot of people, but only online. Lots of people have complained but Sega seems to be ignoring them :/. FSOD (frozen screen of death) and FSOD-X (frozen screen of death-eXtreme) happens when you try to save online, I think FSOD makes you loose all unequiped items not in your inventory, and FSOD-X corrupts your character data.
